we have a tenant who over the last nine months has slowly but surely gone further and deeper in rent arrears now totaling 1,650-00

having tried every repayment plan possible we feel he is using every possible means not to pay.

last week i managed to catch him in we reached a decision that if it was not paid in full he would hand the keys back (but we have been unable to catch the tenant in since after six visits over the weekend with out catching the tenant in.
we have served a section (21)onthe signing of the tenancy agreement he was served with a second one (section 21)in April 2008 giving the two months required

could you please advise on the correct possession forms to be used
