Form N5 or N5B ?

I followed your flowchart diagram. I issued section 8 notice using grounds 8,10 and 12. The rent arrears is more than two months. After 18 days I sent to the court the form N5. But they have sent it back asking me to use N5B. I telephoned the court and thay say N5B is more appropriate for Assured Shorthold Tenancy.The tenancy only 4 months old.
Please suggest which form is correct one.
