Date of new AST, advice please ?

I am a new, small, landlord. l had an 'introduction only' service from a letting agent, including Assured Short-hold Tenancy Agreement for six months , which expired 29 September. Both tenants and I are happy for them to stay, with no changes so am I right that this has 'rolled over' into a Statuary Periodic Tenancy ?

This letting agency then suggested they drew up a new contract for a further twelve months at a fee of 125. However it turns out they wanted 125 EACH from tenants and me, which I thought was a little steep.

I am meeting the tenants tomorrow to sign the new AST, downloaded from the RLA site. Which date should I have on the form ?

Should it be date it is signed, the last day of the month, or a calender month from the end of the previous period ? Does it matter ?

And do we need to have a witness ?
