Disclosure Clauses in Tenancy Agreements

We would like to put a disclosure clause in our tenancy agreement to cover things like:

*change in personal circumstances that the original application/agreement was based on (e.g unemployement/benefit changes etc) for ALL persons living in the property.
*any change in unspent criminal convictions (for insurance purposes) again for ALL persons living in the property

I have searched for examples of these but cant seem to get any and i wondered what the legalities behind it were and how I should word it.

Also, what i can do with the information. For example if a tenant disclosed they had a criminal conviction during their tenancy that would lead to me having to use an insurer with a higher premium, can i pass the cost onto them or draw up a new tenancy agreement with that in (if the original set period has passed).
