If a tenant fails to notify the landlord of maintenance issues, which thus go unremedied for months between inspections and extensive damage is caused..

..and the landlord wishes to document this in the event of supporting eviction if needed,. there a correct way to do this or will a letter (saying what has happened) to the tenant suffice?

I have noted that in some instances you have to use form x,y or z and I wondered if there is a correct way to issue a notice about this to a tenant so that it will be acceptable to a court if needed in the extreme.
