I would like some advice on the right actions to take concerning gaining possession of my property.

It is a multi-let property with four tenants without the need for licensing. I served a s21 notice on the tenants, two have left and one is due to leave in about three weeks.
The last one was given a notice expiring on 5 February 2020. He hasn't paid his January rent and is not picking his phone.
The tenancy commenced in October 2013 and ended April 2014 becoming statutory afterwards.

1) Since the tenancy was before October 2015, am I still okay to send only the gas safety certificate with form N5b to the court?

2) Is it okay to use the property address as my contact address?

3)Does form N215 need to be sent to the court with form N5b?
