Bathroom door lock breaks-tenant forced way out

A recent situation occurred where a tenant's bathroom door lock fell apart.The thumb turn came off and the barrell fell out of the other side.The flat is a 12 months old and she has lived there for 3 months.She was stuck in the bathroom with no window as means of escape and no phone to call for help.She broke the door off the hinges to get out.This has damaged the door frame.

In this situation who is responsible for the damage.As she had to break the door down to get out the landlord should in my opinion pay for it.

A general question is how can landlords ensure that this doesnt happen to them as this could have been a lot worse if she had suffered a heart attack or been injured whilst trying to get out.

Should the thumb turn lock even be in a bathroom where there is no window?.
