abandonment

We have a tenant in an HMO who has caused problems with other tenants.Her behaviour is anti social and threatening and has caused other tenants to leave. we issued a section 21 notice which has not yet expired but we have reason to believe that she has left the property. Most of her possessions have gone, other tenants say she has not been seen for at least 2 weeks and we are told that there have been an number of "people" looking for her. also she will not return or reply to our phone calls. We would like to place an abandonment notice in her room and we are looking for appropriate wording. Do you have a blank proforma we can use? Many thanks.
